Helping individuals and families find, move into, and maintain stable housing.

If you are currently experiencing homelessness or living in a shelter, 1915(i) Housing Support may help connect you with housing-related services through your Medicaid benefits.

This benefit is available through BadgerCare and Wisconsin Medicaid and is designed to support people in finding safe, stable housing and maintaining long-term housing success.

What is 1915(i) Housing Support?

The 1915(i) Housing Support Program helps eligible individuals who are experiencing homelessness or housing instability by providing housing-related support services.

CWCAC is a registered provider through the Wisconsin Department of Health Services and offers services through our offices located in the following counties:

  • Adams
  • Columbia
  • Dodge
  • Juneau
  • Sauk

How Can We Help?

Our Housing Support team works directly with participants to create individualized goals and provide assistance before, during, and after obtaining housing.

Housing Consultation

We work with you to create a personalized housing support plan based on your goals and needs.
Services may include:

  • Identifying barriers to housing
  • Reviewing housing preferences and options
  • Connecting you to additional community resources
  • Creating person-centered goals and support plans

Transition Supports

We help prepare you for moving into housing.
Services may include:

  • Assistance search for available housing
  • Help gathering documents for housing applications
  • Communication and support with landlords
  • Help understanding leases and rental agreements
  • Connecting you with resources needed for housing approval

Relocation Supports

Moving into housing can come with costs and challenges. Assistance may be available for eligible housing-related expenses.
Examples may include:

  • Security deposits
  • Utility activation fees
  • Basic household furnishings
  • Health and safety-related setup expenses

Certain expenses such as rent, mortgage payments, food, moving costs, and household appliances are not covered.

Sustaining Supports

Finding housing is only the beginning. We continue working with participants after housing placement to help maintain long-term stability.
Services may include:

  • Building independent living skills
  • Budgeting and financial organization assistance
  • Tenant education and support
  • Problem-solving and conflict resolution strategies
  • Connections to community resources and services

Who May Qualify?

Eligibility requirements are determined through Medicaid guidelines and approval processes. Individuals may qualify if they:

  • Are currently enrolled in Wisconsin Medicaid or Badger Care
  • Are experiencing homelessness or living in a shelter or transitional housing situation
  • Meet program eligibility requirements
  • Income level: Individual at or below 100% FPL Children at or below 306% FPL
  • Need assistance with maintaining housing stability and supportive services

Eligibility is reviewed individually, and program approval is not guaranteed.

How Does Enrollment Work?

CWCAC may discuss 1915(i) Housing Support services with individuals participating in eligible housing programs, including:

  • Rapid Rehousing (RRH)
  • Tenant-Based Rental Assistance (TBRA)
  • HOME-ARP
  • Youth Homeless Demonstration Program (YHDP)

If appropriate:

  1. We review program eligibility
  2. We complete required paperwork with you
  3. We submit information for Medicaid authorization review
  4. If approved, we begin working together on your housing goals and support plan
